Lower blepharoplasty is a surgical procedure to correct drooping eyelid skin, wrinkles, and bulging under-eye fat. As we age, the skin beneath the eyes naturally sags, but the underlying cause of bulging is often linked to prolonged eye opening, which weakens the orbital septum. The orbital septum supports the eyeball, and when weakened from habits like focusing on screens or wearing glasses for long periods, the fat beneath the eyes can herniate, causing bulging even in younger individuals.

In some cases, the loss of suborbital fat and the recession of the eyeball lead to the formation of wrinkles as the skin loses support. Inadequate sleep, excessive stress, and constant eye straining also contribute to this condition by increasing lymphatic congestion, which causes puffiness and sagging of the under-eye area.

During the procedure, excess skin is removed through an incision under the lashes, or the fat is repositioned via a transconjunctival incision (inside the eyelid). If the fat protrusion is severe, removal or repositioning may be done. However, overly deep incisions can damage facial nerve branches, leading to complications like ectropion (a condition where the eyelid turns outward), which can be avoided by carefully anchoring the orbital septum to the outer bony rim.

To prevent future recurrence, it’s important to focus on maintaining the health of the orbital septum. Practices such as reducing prolonged screen time, wearing glasses less frequently, and regularly practicing eye exercises like gently focusing and closing the eyes can help preserve eyelid strength.
